
These 6 Illinois Nursing Homes Face Fines for Deadly Violations
The Illinois Department of Public Health (IDPH) just released its 4th Quarter report of Nursing Home Violations from 2024, revealing 6 facilities in the state had serious infractions, according to a press release.

These Illinois nursing homes were cited with "AA" violations, the most severe classification under the Nursing Home Care Act, which applies "when a condition or occurrence at a facility directly causes a resident's death."
According to the IDPH report, each facility received a $50,000 fine for the violations.
6 Facilities Cited for the Most Serious Violations
The IDPH's troubling new report on nursing home violations cited six facilities across the state with the most severe penalty.
- Berkeley Nursing & Rehabilitation Center in Oak Park was found to have conditions that led to a resident’s death.
- Elevate Care North Branch in Niles and Elevate Care Waukegan in Waukegan were also cited for "AA" violations, raising concerns about systemic issues within these facilities
- Goldwater Care Gibson City, located in Gibson City, was included in the report for severe violations.
- The Grove at the Lake in Zion was fined for conditions leading to fatalities.
- Morgan Park Health Care in Chicago was fined for conditions leading to fatalities.

Illinois residents with concerns about conditions in nursing homes can file complaints against healthcare facilities licensed by the Illinois Department of Public Health.
